According to the Florida State Plan/Rule: Hospitals shall submit their Medicaid 2552 cost reports no later than five calendar months after the close of its cost-reporting year. Cost reports ending on a day other than the last day of the month are due 150 days after the last day of the cost report period. For those providers filing a final cost report due to a change of ownership (CHOW) or cessation of participation in the Medicaid program, the due date shall be no later than five months following the effective date of either event.
Upon receipt of the Medicaid cost report submission, MSLC will send an e-mail either confirming the submission is complete or requesting additional documentation. After the filing deadline, MSLC will mail certified delinquent letters to providers who fail to file a cost report or fail to provide all necessary documentation.
The Medicaid Hospital Cost Report-Supplemental Information form contains a checklist which specifies the required documentation to submit to MSLC to satisfy a complete cost report package submission. The schedule may be completed and submitted electronically along with the scanned certification signed by an authorized provider representative. This supplemental schedule is where the hospital may supply information regarding its concurrent nursery days, Medicaid DSH (DSR), and indigent care assessment.

The Medicaid Hospital Cost Report-Supplemental Information form is where the hospital may supply information regarding its Medicaid Disproportionate Share Program Key Components (DSR).
Change of Ownership
MSLC should be notified of a change in ownership by letter or by submitting a copy of the notice submitted to CMS or the Medicare contractor. Notification should be submitted to MSLC at the same time as the notification to CMS or the Medicare contractor (copy MSLC on notification).
Change of Cost Reporting Period
The notice of an approved change of cost reporting period received from the Medicare contractor should be sent to MSLC at least 30 days prior to the close of the new cost reporting period.
Cost Report Amendment Procedures
A provider should notify MSLC prior to submission of an amended as-filed cost report due to the difficulty of acceptance after the initiation of the desk review or audit process.
